Vitamin D, Immune System and Chronic Inflammation: What Role Does It Play

The vitamin D is one of the most studied molecules in recent years within the field of immunology and autoimmune diseases. Although traditionally it has been primarily associated with bone health, we now know that its functions go far beyond that. Vitamin D acts as a hormone with an immunomodulatory effect, capable of influencing the activity of the immune system, the inflammation and the integrity of biological barriers, especially at the intestinal level.

The first thing to understand is that vitamin D acts through specific receptors present in many immune system cells, such as T and B lymphocytes, macrophages and dendritic cells. This means that it is not a passive nutrient, but an active biological signal that participates in regulating the immune response. In practical terms, its main function is not to “stimulate” immunity, but to help maintain the balance between an appropriate defensive response and an excessive response that could damage the body’s own tissues.

In the context of autoimmune diseases, this balance is especially relevant. In these conditions, the immune system loses some of its tolerance capacity and begins to react against structures native to the body. Different studies have observed that low vitamin D levels are common in people with conditions such as multiple sclerosis, rheumatoid arthritis, lupus or inflammatory bowel disease. In addition, in some cases, these lower levels have been associated with greater inflammatory activity or more severe clinical symptoms.

However, this relationship must be interpreted with nuances. Vitamin D deficiency is not always the primary cause of the problem. It can also be a consequence of chronic inflammatory state, reduced sun exposure, metabolic disorders, genetic factors that hinder its correct metabolism or the progression of the disease itself.

 

Vitamin D and Immunological Tolerance: The Balance That Prevents Excessive Immune Response

One of the most relevant mechanisms described in the scientific literature is the role of vitamin D in regulating immunological tolerance. This concept refers to the immune system’s ability to recognize self as self and not generate an unnecessary aggressive response.

Vitamin D promotes this balance by modulating different cell populations. Among other effects, it has been linked to increased activity of regulatory T cells and reduced activation of pro-inflammatory subpopulations such as Th1 and Th17, which are involved in numerous autoimmune processes. This effect should not be understood as a suppression of the immune system, but as a more efficient and less aggressive regulation of the immune response.

 

Intestine, Microbiome and Vitamin D: A Key Axis in Chronic Inflammation

Another important aspect is the relationship between vitamin D, intestine and microbiome. The intestine is one of the body’s main immune interfaces, as the immune system is constantly exposed to stimuli from food, microorganisms and metabolites.

Vitamin D participates in maintaining the integrity of the intestinal barrier, promoting the expression of proteins that help keep intestinal epithelial cells bound together. When this barrier is compromised, intestinal permeability can increase and facilitate the passage of pro-inflammatory substances. This can help keep the immune system in a constant state of activation.

In this sense, vitamin D can help preserve a more stable intestinal environment and a better-regulated immune response. In addition, there is a two-way interaction between vitamin D and intestinal microbiome. On one hand, vitamin D can influence the composition and balance of the microbiome. On the other, the microbiome can also modulate the body’s response to vitamin D.

This microbiome-immunity-vitamin D axis is especially relevant in the context of chronic inflammatory diseases, as intestinal dysbiosis can contribute to sustained immune activation.

 

Why Is the Response to Vitamin D Not the Same in All Patients?

Despite these mechanisms, one of the most important points is individual variability. Not all people respond the same way to vitamin D, even when they have similar levels in blood tests or receive equivalent supplements.

This variability can depend on multiple factors, such as the genetics of the vitamin D receptor, the presence of certain polymorphisms, a deficiency of cofactors such as magnesium, the state of the intestinal microbiome, chronic inflammation, body composition, liver function or actual sun exposure. All of this means that its clinical effect depends largely on the biological context of each individual.

In practice, this means moving beyond the isolated number in a blood test. Vitamin D must be interpreted within the patient’s overall state, taking into account their inflammation, intestinal health, lifestyle, diet and metabolic environment. A value within the normal range does not necessarily guarantee a balanced immune response, just as a low value does not always indicate an isolated primary deficiency.

 

Vitamin D Supplementation: A Tool Within a Comprehensive Approach

The vitamin D supplementation can be a useful tool when there is confirmed deficiency, low sun exposure or increased needs. However, it should not be understood as an isolated intervention or a single treatment for autoimmune diseases.

Its role is that of a modulator within a complex system involving many other factors, such as diet, intestinal health, sleep, stress, physical activity, body composition and lifestyle. For this reason, its use should be individualized and, especially in people with autoimmune diseases or active medical treatments, supervised by a healthcare professional.

Overall, current evidence suggests that vitamin D is an important piece in regulating the immune system, with the ability to participate in inflammatory balance, immunological tolerance and intestinal health. It does not explain autoimmune diseases on its own, but is part of the factors that can influence their progression and clinical behavior.

For this reason, assessing vitamin D levels and correcting possible deficiencies can be an interesting strategy within a comprehensive approach, always taking into account the context of each individual and avoiding presenting it as a single solution.
